Researchers propose novel solution to minimize recurrent caries

When patients go to the dentist to fill a cavity, they're trying to solve a problem -- not create a new one. But many dental patients get some bad news: bacteria can dig under their tooth-colored fillings and cause new cavities, called recurrent caries.

She was named the 2016 Poetry Slam Champ of Laredo BorderSlam and has competed in various regional competitions. Last year, she was a member of Houston’s Write About Now poetry team and competed in the National Poetry Slam competition in Denver. She has received media attention for her work illustrating modern life, especially for college-age students and Latinos. Her videos with Write About Now have gone on to be shared by HuffPost, Vibe, Wearemitu, Jorge Ramos and George Takei. A major study has found a marked increase in the number of X-rays when dentists receive payment for them A major study looking into how financial arrangements with dentists affect what goes on in the dentist's chair has found a marked increase in the number of X-rays when dentists receive payment for them. The research, reported today in the Journal of Health Economics , examined extensive data from dentists and patients over a 10-year period and found a significant increase in the number of X-rays given to patients when dentists were paid on a 'fee-for-service' basis, where each item of treatment delivered is charged for, compared to when they are on a fixed salary.

When Dr. Michael Tischler , founder of Teeth Tomorrow met Larry "Smiley" Kleiman, he knew he wanted to help. Smiley's a veteran who did two tours in Vietnam and had almost no natural teeth left when he went in for a consultation with the dentist.

New York dentists show the value of mentorship

As they drove home to New York City from Rutgers University nearly a decade ago, Dr. Marcus Johnson and Dr. Edly Destine may not have imagined they would one day consider each other family. Their relationship is a testament to the power of mentorship, friendship, common interests and following through.

Dr. Johnson, then a third-year dental student at New York University, was participating in a panel of young professionals that Saturday morning. Dr. Destine, considering dental school at the time, was impressed with the humor Dr. Johnson used to answer one question and approached him when the panel concluded. After a brief chat, they discovered that they both lived in New York and rode back to Manhattan together.

Their relationship did not materialize out of thin air, however. “Dr. Destine said he wanted to keep in touch, and he actually did,” Dr. Johnson said. “I thought, ‘This is an individual who follows through and is a person of his word.’”

Dr. Johnson became a mentor to Dr. Destine as he worked his way through dental school, and then offered advice when he wanted to start his own practice after graduation. Now that Dr. Destine is a general dentist and Dr. Johnson is a specialist, they are able to confidently refer patients to each other. And they hang out in New York, where they both still live.

“You cannot live life without a mentor,” Dr. Destine said. “It’s so helpful to have someone to guide you, who knows what you’re looking for in life. All that Marcus has done for me makes me want to pay it forward and help others.”

However, they no longer see themselves solely in the context of a mentor-mentee relationship.

“We’re brothers,” Dr. Johnson said.

Writing the music to your life

Kai Kight: Oftentimes, we go into the school setting looking for a certain outcome — a salary we want to make, a certain job we want. You don’t fully realize that you’re in a place where your No. 1 job is to invest in yourself, to transform yourself. So keep your eyes open to experiences that might not be on that typical, set path because those will differentiate you and provide you with more opportunities. Look for ways that you can learn something new and expand yourself that might not be obvious. And we have access to so many different resources while in school — take advantage of those.

It may be appropriate to re-name the article, as the referenced study does not address root canal success rates. In fact, One might posit the question of whether bond strengths between post and dentin are relevant in determining restorative/structural success, since root fracture does seem to be the more important determinant in post durability.

Caution over lemon water use and dental health

People having the drinks - lemon juice squeezed into a glass of water - are being advised to drink through straws or flush afterwards with plain water to lessen the exposure of teeth to acidic lemon juice. Lemon juice has a pH of around 2. Pure water has a neutral pH of 7 while battery acid a pH of 1. Naturopath Erin O'Hara said the use of lemon water, especially in cleansing routines, has "been around a long, long time".

Social media can certainly help keep you up-to-date with current trends in dentistry, and it’s also a great opportunity for predental students to do some digital shadowing. If you can take five minutes to peruse these accounts or others you find, you may be surprised how the knowledge you gain will be useful one day.

Suburban dentist accused of sexually assaulting woman in his office

A suburban dentist has been arrested and charged with sexual assaulting a woman at his practice in Algonquin, according to police and court records. Algonquin police said the dentist, Eman Shirazi, was taken into custody at his dental office Tuesday and released on bond Wednesday after he was charged with criminal sexual assault, criminal sexual abuse and unlawful restraint.

‘Tri-ing’ something new

January is often a time for new beginnings. Ideas are drafted, goals are set and plans are made. Oftentimes, these goals involve fitness, weight loss or dieting. While these are important factors of health and wellness, they can get old quickly, leaving you feeling bad about falling off the wagon. I am by no means an expert, but I have found that training for something — anything — has made it easier to stick to a resolution.

For me, training for triathlons has been the most fulfilling. As a multi-sport event, training stays variable and less boring. Don’t feel like running? Go for a swim! Don’t feel like swimming? Take out your bike! There are always options that can make workouts less monotonous.

The other positive thing about this kind of training is that it’s social. Doing a workout with a friend is a great way to catch up. Last summer, I trained with a few people from dental school for the Ross Heart Hospital’s Tri-Fit Challenge, an Olympic distance triathlon. Colleagues quickly became friends, and going to workout after class was much easier when someone was going with you. You finally accept your first job at a great private practice. They offer you $600 a day (about $150,000 a year), and you are ecstatic. You remember them saying something about being an independent contractor, but you didn’t really understand what that meant, so you took the job anyway. Now you are left to figure out what that means.

One of the most likely scenarios that’ll ruin a dental grad’s finances is securing your first job as an independent contractor and not understanding the tax consequences. In this post, I’ll give you a general idea of how this could happen and some action steps to prevent an unexpected tax bill.

Most of the time, new grads will be hired as employees. This is usually the case with corporate positions, the occasional exception being with specialists. There are times, though, when new grads find their first job as an independent contractor. What does this mean?

  1. You’re a business owner. You are now a sole proprietor (or a corporation, which I’ll cover in another blog post). You may need to get a business license. Your $150,000 a year is more like “gross revenue” than it is “income.” You are responsible for everything that goes on in your business including paying taxes, having proper licensing, etc.
  2. You can deduct certain business expenses. Since your $150,000 is more like business revenue, it isn’t taxed until after your business expenses. These expenses can be things such as licensing fees, tax and legal fees, equipment purchases, certain insurance costs, etc. If you were an employee who was making $150,000 a year, you would have to pay income taxes on the whole amount. We’ll assume you have $15,000 in business expenses (it’s tough to justify much more that), which means that you will pay taxes on $135,000.
  3. You are responsible for paying taxes. And not just income taxes but also payroll taxes and any other tax your local, state or federal government decides to impose on you. Income taxes typically need to be paid quarterly.

Here’s an example of how this might work.

You make $150,000 your first full year out of dental school. You managed to incur $15,000 in business expenses and are left with a taxable income of $135,000. We will also assume you are in California, which has a high state income tax, and that you are single with no dependents. You will reach the 28 percent federal tax bracket, but because you start out in a lower bracket, your effective tax rate (meaning the actual percentage of your income that went to taxes) will be around 20 percent. For California, the marginal rate would be 9.3 percent with an effective rate of about 7 percent. So you are up to 27 percent of your income going to the government.

The tax that many people forget about is self-employment tax, also known as FICA and Medicare, or payroll tax. If you were an employee, you would pay 7.65 percent and your employer would pay 7.65 percent (which is one reason why many private practice owners prefer to have independent contractors — they don’t have to pay this).

Since you are the “business,” you pay a total of 15.3 percent (12.4 percent for FICA and 2.9 percent for Medicare). Your tax bill is now 42.3 percent (20% + 7% + 15.3%) on your first $127,200 of income, and 29.9 percent thereafter (FICA is only charged on the first $127,200, called the “social security wage base” and can change from year to year). So your tax bill would be roughly $56,000. Sorry for the bad news.

Of course, this is just an example and purely hypothetical. Things such as having dependents could change these numbers. The tax bill could be overstated, as some of the payroll tax is deductible, and you might have other deductions that could reduce your tax bill. Also, I don’t practice accounting, so please make sure you discuss your specific situation with a certified public accountant when it comes to doing your taxes.
